Medical documents from a public or private doctor <\/strong><\/h1>
  • Heart Ultrasound<\/li>
  • Cardiogram<\/li>
  • Opinion of a cardiologist<\/li>
  • Health card signed by a cardiologist<\/li>
  • General urine test<\/li>
  • General blood test <\/li>
  • Opinion of an internist or pediatrician<\/li>
  • For the sport of swimming you also need the opinion of a dermatologist<\/li><\/ul>

    CAUTION!<\/strong> It is not enough to give an opinion on the exams, you need separate <\/u><\/strong> opinion of a cardiologist and a internist. You also definitely need<\/u> <\/strong> a heart ultrasound regardless <\/u> <\/strong>of what appears on the cardiogram. If there are exams in the last 8 months, they are valid<\/u> <\/strong>.<\/p>

    • Completed Registration Application<\/li>
    • Photocopy of ID or birth certificate<\/li>
    • 4 Photos<\/li>
    • PS with genuine signature (GDPR) for the Association
      • of both parents (for an athlete with a mental disability or a minor)<\/li>
      • of the athlete (for adult athlete)<\/li><\/ul><\/li>
      • PS without genuine signature for the Federation<\/li><\/ul>

        Additional documents depending on the Disability<\/strong><\/h1>
        • Athletes with learning difficulties or mental disabilities need a photocopy of the opinion of KEDDY or WISCIII or hospital (if there is, otherwise it will be examined by a partner of the organization).<\/li>
        • Athletes with Down syndrome need a photocopy of a KEDDY or WISCIII or hospital report (if any) and a doctor's certificate that \"does not suffer from<\/em> Atlantoaxial Instability \u00bb<\/em>\u00a0 or that it has \"Asymptomatic Atlantoaxial Instability \u00bb<\/em> and that he can be exercised with a neck x-ray attached.<\/li>
        • Athletes with mobility disabilities need the opinion of a doctor who monitors the athlete to report the condition and difficulties in movement.<\/li>
